This office is in-network with several insurance providers; however, coverage is dependent on specific plans rather than a specific provider. VERIFY YOUR INSURANCE COVERAGE: While our office is pleased to assist you in verifying your insurance coverage, it is ultimately your responsibility to understand what is included in your health insurance policy.
MEDICARE PATIENTS: Medex and Medicare health plans follow Medicare guidelines and aren’t accepted at this practice. Medicare Advantage might be an exception, but coverage isn’t guaranteed. Be sure to review the Medicare guidelines before scheduling. Click HERE for more details.
NEW PATIENTS:Please complete your new patient paperwork online, or plan to arrive 10–15 minutes before your scheduled appointment if you prefer to fill out paperwork in person. Rest assured, any paperwork submitted online is sent securely to a HIPAA-compliant Electronic Health Records portal.
