INSURANCE REMINDER: While our billing company will verify your acupuncture coverage, it is ultimately your responsibility to understand the specifics of your health insurance policy.

MEDICARE: Our practice cannot accept Medicare or MedEx due to specific Medicare guidelines and criteria for acupuncture coverage. This policy applies to all Medex and Medicare supplemental plans. Medicare Advantage plans might be an exception; however, your coverage will need to be verified to confirm eligibility. BERKSHIRE FALLON HEALTH COLLABORATIVE (Out-of-Network): If you are a Berkshire Fallon Health Collaborative member, please be aware that our practice is considered out-of-network. To receive treatment, you must follow these steps:

1. Referral Required: A referral from your Primary Care Physician (PCP) is mandatory and must be sent directly to your insurance company from your PCP.
2. Fax Referral: Please have your PCP’s office fax the referral to Berkshire Fallon Health Collaborative at FAX #: (508) 368-9700.
3. Insurance Approval: Your insurance company requires this referral to approve or deny your out-of-network coverage for acupuncture.
4. Appointment Scheduling: Once we receive an approval letter from your insurance company, we will contact you to schedule your appointment. Please note that this approval or denial process can take up to 14 days.

MASSHEALTH INSURANCE (THIS IS NOT FOR A MASSHEALTH CONNECTOR PLAN): A referral from your Primary Care Physician (PCP) is required to receive treatment in this practice.
